After-Hours Physician
To speak with a Catawba Valley Medical Group provider afterhours, please call: 828.324.0258. This service provides direct access to the on-call provider, not a triage phone line, to increase access to care and is intended for medical questions or concerns. Please call 911 if you are experiencing a medical emergency.

Weekend After-Hours Care:
The convenience of extended office hours is provided by Catawba Valley Family Medicine - Maiden to all Catawba Valley Medical Group patients on Saturdays 9am – 1pm. Click here for more information.

Patient Portal:

Deji Okuboye, DO

Dr. Deji Okuboye received her degree in Osteopathic Medicine from Nova Southeastern University in Davie, Florida. She completed her internship and residency in Family Medicine through Palmetto General Hospital in Hialeah, Florida. Additionally, she is a licensed Physical Therapist. Originally from Clearwater, Florida, Dr. Okuboye enjoys international travel, competes in long distance running, and spends free time with her husband and daughter.

April Phares, FNP


portraitApril Phares is a Board-Certified Nurse Practitioner in family medicine. She graduated from South University in High Point, NC., and received her undergraduate degree in nursing from UNC-Greensboro in Greensboro, NC. April was a bedside nurse for 16 years prior to graduating with her nurse practitioner degree. She enjoys spending time with her family, shopping for antiques, running 5/10k races, and taking care of her animals, both farm and domestic.

Sandra Reed, FNP

Sandi Reed is a board-certified Family Nurse Practitioner, who earned her master’s degree in nursing from Western Carolina and received her undergraduate degree in nursing from UNC-Wilmington. She has worked in the local community, prior to joining the CVMC family, where she has appreciated building wonderful relationships through nursing. Sandi is passionate about effectively educating patients about sustainable lifestyle interventions to ultimately promote healthier families and thriving communities.

In her free time, she enjoys designing anything creative, volunteering at church, baking, and spending quality time with her family.
